Thank you for your interest in this research study; your contribution is very important. The study is being conducted by PEGUS Research.

To see if you qualify to participate now, you need to answer a few questions and provide some basic contact information. This process should only take about 5 minutes. If you qualify, this study will include a short 10-15-minute video interview to complete one additional eligibility step. If you qualify after the additional eligibility interview, you will be scheduled for the study interview appointment (also done via a video interview) which may take approximately 90-120 minutes. The video interviews will require you to use a computer, tablet, or smart device with a video camera.

The information you provide is used strictly for the purposes of the study and is stored in secure databases that are only accessible to our employees and other study staff who have a need to know this information. Demographic information you provide to us may be shared with the study sponsor or regulatory agencies, but it will always remain anonymous unless otherwise required by law. If you do not qualify to participate, your personal information retained by PEGUS Research will be destroyed at the end of the study.

Please note that if you are 16-17 years of age, or if you reside in Alabama or Nebraska and are 18 years of age, or Mississippi and are 18-20 years of age, you will need to have a parent or legal guardian available during the interview to provide permission for you to participate in this study.
